I did something similar for my twin sister this year. I made a "sweater request form" for her birthday present. It was a long, thin form like a sushi order form, but instead I made it with check boxes for style of sweater (pullover, cardigan), neck (turtle, mock, crew, V), and every other option I could think of including pattern and color. She loved it and my mother was instantly jealous! I'll be making a cabled long sleeved cardigan for my sister, as soon as she decides on a color.
